Atrial Fibrillation Virtual Clinic webinar

Purpose: To support and provide information to those CCGs involved in the NHS England Atrial Fibrillation Patient Optimisation Demonstrator Programme (AF virtual clinic project)

Understand the need to improve anticoagulation rates for people with Atrial Fibrillation.

Learn about the AF Virtual Clinic model from Lambeth and Southwark CCG.

Hear why NHS England are funding this project.

Find out how the AHSNs are supporting clinical commissioning groups to deliver this project.

Key points for commissioners including contracting, finding your specialist pharmacist, GP engagement, understanding the anticoagulation pathway, practical issues in delivery.

Define expectations of participating clinical commission groups, including data collection and reporting, use of funding, reporting timeframes, waiting times for a/c, project evaluation.
